How does remote monitoring work?

For remote patient monitoring, we have patients use a tablet, blood pressure cuff, pulse ox and scale to provide vital information at least once a day. Our nurses receive these results and contact the patient or their physician if needed to address any concerns. We’ve found success with this program by reducing hospital readmissions and giving patients peace of mind.
